Authorization
I have read the separate Background Check Disclosure, on its own, before this authorization.
I authorize Muslim Red LLC, a Wyoming limited liability company ("Muslim.Red"), to obtain consumer reports about me, for employment purposes, from [the consumer reporting agency's name, address, toll-free phone and website], to decide whether I may deliver through Muslim.Red Delivery as an independent contractor.
What happens with the report
- Muslim.Red orders no report until I have read the disclosure and signed this authorization.
- A report never turns me away by itself. A person at Muslim.Red reads any result that is not clear, and either clears it or writes to me before deciding.
- Before any decision against me based on a report, Muslim.Red gives me a copy of the report and A Summary of Your Rights Under the Fair Credit Reporting Act, tells me what is being weighed and why, and waits at least 5 business days for anything I want it to know.
- Muslim.Red keeps the agency's reference and the result, never the report itself, except a copy it gives me with a notice, which it deletes if the result is cleared.
I sign by typing my full name in the driver app. A copy of this authorization stays at this page's address, and I may ask Muslim.Red for the version I signed by writing to support@muslim.red.
